Grant application
The Millbrae Community Foundation uses a formal grant application process to determine grant requests/proposals. Applying for a grant requires a significant amount of effort on the part of the applicant. The Foundation recognizes that fact and assures you that the information requested is not only necessary, but is also critical to the overall process.
Proposals should be no longer than three pages (excluding covers and attachments) and must follow the specified outline.
Information requested includes an Executive Summary, Program/Project Description, Organizational Description, Evaluation Process, and Financial Information.
Completed applications/proposals must be received no later than 5:00pm on June 30th (or by 5:00pm on the Friday proceeding June 30th if June 30th falls on a weekend).
