Proposals should be no longer than three pages (excluding covers and attachments) and must follow the specified outline.

If you are unable to download a form, please fill out the Information Request Form below and a Grant Application and/or Final Report form will be sent to you via e-mail or mail.
Final Report Form:
Applicants are also expected to report to the Foundation after their grant money has been received and utilized. We are always excited to see the results of our endeavors.

The Millbrae Community Foundation uses a formal grant application process to determine grant requests/proposals. Applying for a grant requires a significant amount of effort on the part of the applicant. The Foundation recognizes that fact and assures you that the information requested is not only necessary, but is also critical to the overall process.
